Module 4 discusses how to create a data-driven aim statement. The aim statement data plan is the guiding light of all successful change projects. The most important aspects of developing your data plan are ensuring your change team keeps the rapid-cycle change approach central to the NIATx model and using NIATx tools to develop an achievable, comprehensive plan to reach your desired aim. In this module, we cover how to choose the right indicator measures, gather baseline data, and set stretch goals that motivate your team.

ACTION ITEM: Access the NIATx Change Project Charter and the NIATx Tools and Resources, so you are prepared to develop your aim statement data plan.
