New rule. Any patient w/suspected or confirmed COVID-19 who needs intubation gets a central line and an arterial line at the same time. This is to reduce in and out PPE use as well as make it easier for the RTs to get the blood gases and the nurses to maintain access. Opinions?

As a medicine-trained intensivist, I did not get too much into the weeds of base excess and base deficit. Well, here’s a good 3-page breakdown. 🎩 tip to the authors. eddyjoemd.com/foamed/

Hypertensive emergencies can shred organs. Patients can present with various clinical manifestations. As much as we may want to make the numbers pretty immediately, we may need to show restraint. Here's how to do it. 🎩 tip to the authors. eddyjoemd.com/foamed

One cannot be a master of blood gases if they do not understand base deficit/excess. Here’s a deep dive. 🎩 tip to the author. eddyjoemd.com/foamed/

Procalcitonin's role in ruling in or ruling out bacterial infection is overrated. However, its potential for guiding the duration of therapy may be under appreciated and underutilized. 🎩 tip to the authors. eddyjoemd.com/foamed/

Hyperkalemia management: No supporting clinical effect was identified for calcium or bicarbonate. Is it time to rewrite all the order sets? 🎩 tip to the authors. eddyjoemd.com/foamed

Our colleagues in Japan have released their sepsis guidelines. They suggest bicarbonate for a pH < 7.2. How do you feel about this? 🎩 tip to the authors including @YukiKotani5 eddyjoemd.com/foamed

Norepinephrine is such a powerful vasopressor due to its effects on both cardiac output and SVR to increase the MAP. This image depicts how it affects the CO. 🎩 tip to the authors including @DrMiguelIbarra1, @edu_kattan and @cjungMD. eddyjoemd.com/foamed/

Euglycemic DKA is becoming more commonplace in our ICUs. I recall the first time I was called to admit a patient with this. I asked “what is that?!?” This paper addresses the mechanisms, manifestations, diagnosis, and treatment. 🎩 tip to the authors. eddyjoemd.com/foamed/

The decision as to whether we should provide bicarb to our critically ill patient takes places every single day. This paper breaks things down amazingly well. 🎩 tip to the authors. eddyjoemd.com/foamed/

IVC assessments were the “hot new thing” in POCUS when I was a fellow. Today, it’s a very small component of my practice. There are too many variables involved to help one make a volume status decision based solely on this. 🎩 tip to the authors. eddyjoemd.com/foamed/

Vasopressin is extremely powerful and not only meant to be used as a “backup dancer” as I refer to in my book, The Vasopressor & Inotrope Handbook. This paper provides a free deep dive into the data. 🎩 tip to the authors. eddyjoemd.com/foamed/
