Pups at the Pitch Registration + Waiver
Union Omaha is excited to have you and your dog(s) out to our "Pups at the Pitch" match on Saturday, May 23rd! You can purchase tickets for the 7pm match here: https://tinyurl.com/UOPupsAtThePitchTix26  

Please read through the paragraph below, and answer all questions.

DOG(S) MUST SIT WITH THEIR OWNERS ON THE BERM.  Dog(s) are allowed to walk the concourse before and during the match.  Again, your furry friends are welcome to join their owners on the berm area located on the east sideline. They are not allowed in the seats or the supporters' section on the south side of Morrison Stadium. No cats or caged animals will be allowed into the stadium.

BRING BAGS FOR WASTE CLEAN UP. 

In order to bring your dog(s) into the stadium, you must register each dog separately using this form. Once registered, please see the table outside of gate one in order to receive a ticket which allows your pet(s) to come into the stadium.

In consideration of being allowed to participate in any way with the activities of any kind, and bring my pet(s) into Morrison Stadium, located at 1804 California Street, Omaha, NE, I KNOWINGLY AND FREELY ASSUME ALL RISK - both known and unknown - for any and all injuries and claim responsibility for myself and my own animal(s). There is an element of risk for injury in any participation. My behaviors and actions, as well as the behavior and actions of my pet(s), are my own. Risk of injury increases should I not follow the rules or instructions implemented by the Alliance Omaha Soccer LLC and Union Omaha. 

Alliance Omaha Soccer LLC and Union Omaha, Creighton University and all of their assignees are hereby released, indemnified and held harmless from any and all claims arising out of my participation at said event.
