Nursing homes and assisted living facilities that provide memory care and offer protected, secure areas that are locked and monitored so that residents with dementia can move around safely without accidentally leaving the property.

Programs that maintain information about retirement residences, residential care facilities and nursing homes, and link older adults who are looking for alternative living options with appropriate independent or supervised living resources. ADRD Care Partner & Family Member Resources

Respite Care Services

Respite care gives care partners/caregivers a much-needed break. Your loved one will receive safe, professional care while you step away to rest, run errands, or take a trip. This can be done at home or at a nearby care center. Research consistently showed that respite improves the quality of life of both the caregiver and the person living with dementia. Care partners either pay for these services out of pocket or work with a public agency for free respite services.

Guiding an Improved Dementia Experience (GUIDE) Model

The GUIDE Model focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. GUIDE is available for Medicare Fee for Service/Traditional Medicare insurance holders. GUIDE Providers are available to assist in connecting to services and supports.

A care partner is an unpaid caregiver, typically loved ones, family members, or friends, who provide care to someone living with ADRD.

Support Groups

Support groups for loved ones living with dementia and caregivers and family members of someone living with dementia.

Adult Day Programs

Offers individuals living with ADRD the opportunity to socialize and participate in activities in a safe environment outside of the home at a senior center, community center, or local organization with trained dementia care professionals.
