Patient portal
The FollowMyHealth patient portal is used with your Northwell account to give you more access to your personal medical records—from Northwell and other medical providers—and more control over your health care.
With the patient portal, you can:
- View prescriptions and lab results
- Access notes from doctor appointments
- Email your physician directly
- View discharge instructions after a hospital stay
Medical records
As a patient, you have the right to access your medical records. Copies of medical records are available to patients and physicians with the proper authorization. Our Health Information Management Office can help—call (212)/(516) 562-4260.

Amputee support group

Circle of Friends offers education, support and resources for brain aneurysm survivors. Family members, caregivers and all others interested are welcome to attend.

Access to health services is provided for those with communication needs or impairments.
We provide you with the medical attention and nursing care you need to ensure a rapid and safe recovery.
We offer a free referral and assistance center to help seniors and their families find and access services.
Patient satisfaction
Our top priority is providing you with the very best care. About two weeks after you leave the hospital, we will send you a questionnaire regarding the care you received at North Shore University Hospital. Please take a few moments to complete and return it in the envelope provided. Your feedback is extremely valuable and will help us continue to improve the quality of care for all patients.
You may also be selected to participate in the Hospital Consumer Assessment of Health Providers and Systems (HCAHPS) survey after your discharge. This survey, backed by the U.S. Department of Health and Human Services, asks multiple-choice questions about your hospital stay and measures your satisfaction with the quality of your care. It is designed to be a standardized tool for measuring and reporting satisfaction across all hospitals in the U.S.

Appealing a discharge decision
Your physician determines when you are ready to be discharged. Remember, being discharged doesn’t necessarily mean that you are completely well—it only means that you no longer need hospital services. If you disagree, you or your representative can appeal the doctor’s decision.
If you are a Medicare patient, be sure you are given a notice called An Important Message from Medicare by the hospital’s discharge planner or caseworker. This details your rights to remain in the hospital for care and provides contact information to appeal a discharge decision.
